Dementia care management in routine care using the example of the Siegen-Wittgenstein region
The project focuses on Dementia Care Management (DeCM) as a measure variable of the National Dementia Strategy. Based on the already positively evaluated DelpHi standard for the outpatient care of people with dementia, the development and implementation of a regional and cross-sector DeCM model for the Siegen-Wittgenstein region (SW) is being investigated. Following a participatory research approach, practitioners from various care sectors as well as people with dementia and their relatives from SW are involved as co-researchers in all phases of the study.
Project description
Dementia is one of the main causes of disability and dependency in older people worldwide and is reflected in the economic, social and, above all, care-related consequences of the disease. This is a challenge for society as a whole that must be overcome in the coming years, and in RoutineDeCM, Dementia Care Management (DeCM) has been implemented by project partners in the Siegen Wittgenstein region as a model since July 2022. Specifically qualified specialists use computers to identify the medical, nursing and psychosocial (care) needs of people with dementia and their (caring) relatives in their home environment. On this basis, the dementia pilots create an individual care plan in cooperation with all those involved. The implementation process is carried out in a participatory manner by researchers from the University of Siegen and the DZNE, in close cooperation with those affected, various care providers and cost bearers from different areas as well as the developers of the system.
