Decision support for advance health care planning for people with dementia

The DECIDE-2 project aims to provide decision support for advance health care planning for people with dementia and their relatives in order to strengthen the autonomy of those affected when making decisions for the last phase of life and to reduce the burden on relatives and caregivers.

The project focuses on Dementia Care Management (DeCM) as a measure variable of the National Dementia Strategy. Based on the already positively evaluated DelpHi standard for the outpatient care of people with dementia, the development and implementation of a regional and cross-sector DeCM model for the Siegen-Wittgenstein region (SW) is being investigated. Following a participatory research approach, practitioners from various care sectors as well as people with dementia and their relatives from SW are involved as co-researchers in all phases of the study.
